Our patient portal online tool is called MyChart. This is service we purchase from Sanford Health to give you online access to your health record. When you sign in, or sign up as a new user below, you will be directed to a safe MyChart website.
Your records stay within McKenzie Health.
USING MYCHART
Video Visits Tutorial
Meeting with a provider by video visit is a convenient way to get care without coming to the hospital or clinic in person.
You can use a computer or smartphone to start a video visit. Using your smartphone is the easiest option because it already has a camera and microphone built in. To get started, download the MyChart Mobile app from the Apple App Store or Google Play store.

How to Join the Next Available Video Visit
1. Log in to the MyChart app on your smartphone and select On-Demand Video Visits from the menu.
2. Confirm the reason of your visit is NOT an emergency and then confirm the state you’re currently in. If your smartphone asks to use location services, allow it.
3. Select your reason for visit from the list of options.
4. Click okay to test hardware and make sure the microphone, camera, and speakers on your smartphone work.
5. Select Put Me In Line to be seen by the next available provider if it’s available or you can choose to Schedule a time that is most convenient for you.
Schedule a Future Video Visit
- Log in to the MyChart app on your smartphone and tap the Visits button on the home screen.
- Select the Schedule an appointment button.
- Scroll down the menu to find and select On-Demand Video Visit.
- Confirm the reason of your visit is NOT an emergency and then confirm the state you’re currently in. If your smartphone asks to use location services, allow it.
5. Select More Times to select an appointment time for your video visit.
6. Enter relevant information in the comments section and then click Complete.
Join a Scheduled Video Visit
- At least fifteen minutes before your video visit appointment, log in to the MyChart app on your smartphone and choose the Visits icon on the home screen.
- Your scheduled video visit appears in this list. Tap it to open the details.
- Tap Online Check-In to complete the necessary steps before starting your visit.
- If asked, allow your browser to access your smartphone’s microphone and camera.
- Tap Test hardware to ensure your camera, microphone, and speakers are working.
- Tap Join video visit.
- Wait for your provider to join. They will be will you soon!

Connie Wold Wellness Center
The Connie Wold Wellness Center was constructed in 2012 as a replacement for the Healthy Hearts Wellness Center facility. The facility hosts a variety of workout equipment, indoor running/walking track and two indoor endless pools. The center also houses the Physical Therapy department of McKenzie Health and Connie’s Cup, a coffee shop in the Wellness Center Lobby.

Horizon Assisted Living has 1 and 2-bedroom Senior Living apartments for Rent! The Horizon is connected to the MCHS Good Shepherd Home, Hospital and Clinic Campus. The Horizon offers seniors the chance to live as independently as possible, providing dinner daily. Additional assistance is available on a fee for service basis.
