What is a CHNA?
A Community Health Needs Assessment (CHNA) is a written report that identifies and analyzes the health needs of a community to inform decision-making.
The assessment includes data on
– demographic characteristics,
– risk factors,
– health-promoting and preventive services,
– environmental factors,
– health care resources, and
– existing conditions within the community.
The CHNA is an important tool that can help communities identify key areas in need of improvement. It also serves as a guide to develop effective strategies to improve health outcomes and prevent disease in the future.
